Job summary

ICFAI in Statesville, NC seeks a Program Manager to oversee a high-quality Diabetes program, supervise staff, and direct all facets of the program. You will provide leadership, education, and self-management materials to participants, patients in rehab, and community members while maintaining HIPAA compliance.

The role requires a strong diabetes education background, possible CDE certification, and experience in program budgeting with collaboration across the wellness team.

Qualifications

• Associate or bachelor’s degree is required.
• Current RN licensure in NC or RD to practice in NC.
• Strong background in diabetes education.
• Prior managerial experience is preferred.
• Certified Diabetes Educator with current certification is highly preferred.
• Completes a minimum of 15 hours of continuing education in diabetes, education principles, and behavioral strategies every year preferred by ADA.

Responsibilities

• Receive referral information and schedule an appointment for the participant.
• Obtain medical history and clinical needs assessment of assigned participant.
• Deliver appropriate diabetes management education and training based on assessment results.
• Lead group classes and tailor education to individual needs with HIPAA adherence.
• Deliver education and training to participant and others in outpatient, community and corporate settings.
• Document information related to Diabetes training and patient progress in the medical record.
• Maintain patient information confidentiality and communicate with the health care provider about patient training.
• Provide follow-up services to participants who completed training.
• Assess needs for weight management education and promote to enhance outcomes.
• Assess lifestyle and dietary habits, determine dietary restrictions, and make recommendations.
• Participate in community education programs and marketing activities as requested.
• Ensure ADA Recognized status maintenance and continuous program CQI.
• Understand Joint Commission standards and update training manuals accordingly.
• Maintain adequate staff and conduct performance evaluations.
