Job summary

Pueblo of Sandia seeks a capable Diabetes Education Program Coordinator to lead a community diabetes initiative. The role includes education, prevention, and collaboration with health staff to serve the Pueblo population.

The position emphasizes program development, KPI tracking, and grant reporting, with an expectation to coordinate across departments and conduct impactful wellness activities.

Qualifications

• Knowledge of diabetes and healthcare prevention programs including physical activity, nutrition and diabetes management.
• Ability to travel and attend training seminars, continuing education programs, meetings and conferences.
• Basic computer knowledge using Windows based computer and Microsoft Office software.
• Experience working with children and seniors.
• Ability to give oral presentations and prepare written reports.
• Ability to collect, analyze and report data.
• Knowledge of Indian Health Service and other health services.

Responsibilities

• Plan, designs, implements and assess a systematic comprehensive diabetes program.
• Writes, manages, and reports on State/Federal grants as appropriate.
• Develops and manages annual budget.
• Establishes strategic preventative health goals for all ages.
• Creates marketing and outreach plans for the diabetes program.
• Develops KPIs to track program goals.
• Provide education on diabetes care, self-care and prevention.
• Coordinate diabetes care with Clinic/Wellness departments.
