Job description

This is a non-benefited, per diem/casual position with day shifts. The schedule will be around 10-20 hours per week, between 7am and 7pm, Monday through Friday. It is a work-from-home role, requiring residency in San Diego, with some training possibly on-site.

Position Overview

The Diabetes Care Manager - RN will oversee care management for diabetic pregnant patients, with responsibilities including:

  • Managing a panel of high-risk patients independently, in collaboration with endocrinologists and perinatologists.
  • Providing clinical education and management, including insulin titration and pump adjustments under physician guidance.
  • Entering medication and supply refill orders as needed.
  • Assessing patient needs and coordinating referrals to other care team members.
  • Participating in care coordination conferences with provider partners.
  • Developing population health strategies for high-risk groups.
  • Supporting transitions of care across ambulatory and inpatient settings.
Qualifications

Required Education/Experience/Specialized Skills:

  • At least 3 years of experience as an RN in endocrinology, perinatology, obstetrics, women’s health, diabetes, or community clinics.

Required Certification/Registration:

  • RN license and CDCES certification.

Preferred Qualifications:

  • Bilingual in English and Spanish.
  • Insulin Pump Trainer certification.
  • Experience managing patients with complex diabetes, including technology like insulin pumps and glucose monitors.
