Job summary
CT Nurses is seeking an RN Diabetes Care Manager to design individualized diabetes self‑management plans, interpret clinical data, and coordinate care for chronic patients. This role collaborates with patients, families and the healthcare team to improve outcomes and quality of life.

Qualifications
- Graduate of an accredited Bachelor of Nursing Program.
- Registered Nurse with an active license to practice in State of CT.
- Five (5) years of clinical practice.
Responsibilities
- Create individualized diabetes self‑management action plans.
- Interpret clinical data to support patient care decisions.
- Conduct focused or comprehensive educational assessments.
- Coordinate care for chronic patients across the health system.
- Collaborate with patients, families and healthcare teams to improve outcomes.
