Job summary

Milestone is seeking a dedicated care staff member in Loves Park, IL to support individuals with developmental disabilities. You will perform personal care tasks, participate in activity therapy programs, and follow daily lesson plans under supervision.

Responsibilities include documenting health events, ensuring safety during activities, and assisting with daily living skills while maintaining a positive, respectful environment for the individuals served.

Qualifications

• Must be over 18 years of age.
• High school diploma or GED.
• Must pass the TABE test.
• Must be able to intervene physically in behavioral and health related crisis with individuals.
• Must complete habilitation aide certification.
• Nurse’s aid experience or some personal care experience preferred.

Responsibilities

• Intervenes on behalf of individuals served and reports to the supervisor or relevant agencies.
• Maintains attendance and participates in shift change reports to ensure coverage.
• Monitors individuals’ health needs and reports changes promptly to nursing staff.
• Provides ongoing active treatment per individual plans and documents progress.
• Notifies and records incidents and injuries, maintaining accurate charts and logs.
• Assists with daily living activities and reinforces treatment and development programs.
• Ensures safety and uses appropriate equipment for transfers and mobility as trained.
