Adaptive Home Health, LLC is seeking a Home Health Aide to deliver compassionate in-home care in Detroit. You will manage a full caseload, coordinate with the care team, and help patients maintain independence in their homes while building trust with discharge planners and physicians.

This field-based role offers scheduling flexibility, mileage reimbursement, and the chance to contribute to an AI-native platform expanding access to quality home health care.

Qualifications

• Must have Michigan HHA certification.
• 1+ year of experience in personal care.
• CPR/BLS certification required.
• Valid driver's license and reliable transportation.
• Ability to lift and transfer safely.
• Strong communication and professional demeanor.
• Comfortable working independently in patients' homes.
• High school diploma or GED.

Responsibilities

• Provide personal care including bathing, dressing, grooming and toileting.
• Assist with transfers, ambulation and mobility using proper body mechanics.
• Support medication reminders per care plan.
• Monitor patient condition and vital signs; report changes to supervising nurse.
• Perform light housekeeping to ensure safe home environment.
• Document care accurately in HCHB.
• Communicate with patients, families and care team for continuity of care.
• Follow infection control practices and preserve patient dignity.
• Assist with range of motion exercises as ordered.
• Coordinate with supervising RN/LVN for care plan updates.
