Job summary

Beth Israel Lahey Health’s Danvers Treatment Center is seeking a dedicated Case Manager per diem to support adult clients facing addiction. You will conduct intake assessments, coordinate referrals, and assist with continuity of care in a patient-centered setting.

You will collaborate with clinical staff to plan treatment and ensure smooth transitions to aftercare resources. A background in human services and a genuine commitment to recovery are essential.

Qualifications

• Associates or Bachelor’s Degree in Human Services or related field preferred; High School Diploma or Equivalent required.
• Minimum of one year related experience and/or training in addiction treatment.
• Valid Driver’s License in good standing preferred.

Responsibilities

• Obtains documents and all relevant intake material; conducts intake evaluation and counseling sessions.
• Shares responsibility for education and other group programming activities.
• Participates in client evaluations and case conferences.
• Provides referrals for affected family members to self-help programs when applicable.
• Assesses continuing care needs, arranges transfers/admissions to rehab facilities; liaises with self-help organizations.
• Coordinates transportation to the next level of care.
