Job summary

Beth Israel Lahey Health (BILH) is seeking a Case Manager for Danvers Treatment Center. The position supports adults in detox with 24-hour nursing oversight and coordinated case management, education, and group programming.

The role involves intake evaluation, developing continuing care plans, arranging transfers to rehab or self-help resources, and coordinating transportation. A related degree is preferred; one year of addiction-treatment experience is expected.

Qualifications

• Associates or Bachelor’s Degree in Human Services or related field preferred; High School Diploma or Equivalent required.
• A minimum of one year related experience and/or training in the area of addiction treatment.
• Valid Driver’s License in good standing preferred.

Responsibilities

• Obtains documents and intake materials; conducts individual intake evaluation and counseling sessions.
• Shares responsibility for education and other group programming activities.
• Participates in client evaluations and case conferences.
• Provides referrals for affected family members to self-help programs when applicable.
• Assesses patients’ continuing care needs; arranges transfer/admission to appropriate facilities; liaises with self-help organizations.
• Coordinates client transportation to next level of care.
