Job summary

Beth Israel Lahey Health's Clinical Stabilization Services (CSS) offers a 40-bed detox program for men and women with substance use histories. It provides short-term intensive treatment with individual and group counseling, risk assessment, case management and after-care planning.

The Case Manager role supports assessments, formulates treatment plans, assists in discharge planning and coordinates transfer to appropriate care levels with after-care resources, while participating in staff meetings

Qualifications

Associates or higher in Human Services or related field preferred; HS diploma required.
Minimum of one year related experience in addiction treatment.
Valid driver's license in good standing preferred.

Responsibilities

Participates in assessments and assists in formulating treatment plans.
Assists in discharge planning and arranging transfer or admission to appropriate care levels.
Attends staff meetings and client evaluations, case conferences and group activities.
Counsels clients individually and in groups; monitors progress and adjusts treatment as needed.
Refers clients to medical, social and employment services as needed.
Prepares and maintains reports and case histories.
