Qualifications

• Master's Degree in Social Work, Registered Nursing with behavioral health experience, or a related field.
• Minimum of 2 years of experience in a behavioral health setting.
• Proficiency in community resources and in coordinating with diverse service settings.
• Strong communication, motivational interviewing abilities, and computer literacy (MS Word, Excel, Outlook).

Responsibilities

• Consult with patients and primary care providers to discuss treatment options and develop person-centered treatment plans.
• Provide information and referrals to connect patients with community services and scheduling assistance.
• Conduct assessments, including PHQ-9, and gather depression histories.
• Coordinate depression treatment plans with providers and behavioral health professionals.
• Arrange urgent care evaluations and ensure access to care.
• Monitor psychosocial needs and provide problem-solving support.
