Job summary

Enterprise Medical Recruiting seeks a compassionate Nurse Practitioner to join its Denver-area team with a growing physician-led medical group serving Skilled Nursing, Long-Term Care, Assisted Living, Independent Living, and Rehabilitation settings.

You will provide medical care and follow-up in post-acute care, collaborate with physicians, therapists, case managers and facility staff to coordinate patient care, complete timely clinical documentation, and participate in quality improvement

Qualifications

• NP with Family, Acute Care, or Adult Gerontology certification.
• Active Colorado NP license or ability to obtain.
• Post-acute care, geriatrics, family medicine, internal medicine, or skilled nursing experience preferred; new graduates considered.

Responsibilities

• Provide medical care and follow-up in post-acute care settings.
• Collaborate with physicians, therapists, case managers, and facility staff to coordinate patient care.
• Complete timely and accurate clinical documentation.
• Participate in quality improvement initiatives and ensure regulatory compliance.
• Travel to assigned Skilled Nursing, Assisted Living, and Independent Living facilities throughout the Denver metro area.

Education
