Community Health Center Inc. maintains strong HPSA scores of 19 for primary medical care, 21 for mental health, and 25 for dental services. Provides oral prophylaxis, scaling and root planing, sealants, dental and periodontal charting and topical preventive treatments for patient population. To meet productivity targets quarterly2. Provides appropriate individualized patient education for maintenance of oral health. Emphasizes preventive techniques and compliance with recall intervals.3. Provides radiographic services.4. Reviews patient medical history prior to start of visit. Maintains accurate documentation of dental hygiene services provided and a hygiene assessment including periodontal charting by completion of the electronic dental record at the end of each visit.5. Maintains the treatment area, equipment, instruments and supplies in proper working order daily. Strictly follows safety and infection control standards.6. Shares responsibility for the maintenance of the log book required documentation and task list including the autoclave with the dental assistant at a fixed site.7. Ensures all electronic charting, documentation, and telephone encounters are in compliance with agency standards.Administrative1. Participates in assessment of dental services and quality assurance program.2.
