Job summary

Memory Care Home Solutions in Missouri is seeking a Social Worker/Dementia Navigator to support people living with dementia and their families. The role provides home-based, telehealth, and in-center services, offering education, resource connection, and ongoing guidance in a compassionate, evidence-based approach.

You will report to the Director of Programs and collaborate with healthcare professionals and community groups.

Qualifications

  • Bachelor of Social Work or equivalent degree or work experience.
  • Minimum two years of post-degree experience in geriatrics.
  • Experience with people from diverse cultures and socio-economic backgrounds.
  • Excellent verbal communication and time management skills.
  • Knowledge of basic healthcare and social service resources.
  • Attention to detail and creative problem-solving skills.
  • Proficiency in MS Office and experience documenting in EMR systems.
  • Provide proof of CPR Certification and maintain it.

Responsibilities

  • Complete assessments of clients using a formalized assessment process.
  • Deliver in-person and telehealth dementia intervention services and follow-up using evidence-based protocols.
  • Develop care plans to address clients’ psychosocial barriers to health and wellness.
  • Provide dementia education to people of diverse backgrounds and literacy levels.
  • Coordinate care and communicate with service providers and physicians.
  • Meet assigned productivity expectations for navigation services.
  • Submit thorough and timely documentation within 24 hours of service delivery.
  • Develop and maintain resource files and network with other service providers to assist clients.
  • Track needed data and complete benchmarking tools.
  • Identify and present to community groups about dementia education and support.
  • Build partnerships with agencies to build referral pathways.
  • Develop educational presentations for health care and senior service professionals.
  • Be available for some evening and weekend work for presentations and consultations.
  • Support grant or funding initiatives with success stories and testimonials.
  • Contribute to monthly web/social media education.
  • Participate in quality improvement and staff development activities.
  • Perform other special projects and duties as assigned.

Position Overview

The Social Worker or Dementia Navigator provides compassionate, evidence-based support for people living with dementia and their caregiving families, helping them navigate the challenges of the dementia journey and access the resources they need to thrive. This role delivers services in the community, via telehealth, and at Memory Care Home Solutions’ headquarters and specialized caregiver training environment. In this position, the Social Worker or Dementia Navigator addresses mental health and caregiving challenges specific to dementia, provides dementia-specific education, connects clients with community resources, and offers ongoing guidance and support as needs evolve over time. You’ll also provide educational outreach to healthcare professionals and community groups, helping to expand understanding and awareness of dementia care in the broader community. Reporting to the Director of Programs, this is a full-time (40 hours/week) role. This position is ideal for a mission-driven professional who is enthusiastic about relationship-based care, education, and empowering families and individuals living with dementia.

Key Performance Objectives and Responsibilities

90% – Dementia Navigation

  • Complete assessments of clients using a formalized assessment process.
  • Deliver in-person and telehealth dementia intervention services and follow-up using evidence-based protocols.
  • Develop care plans to address clients’ psychosocial barriers to health and wellness.
  • Provide dementia education to people of diverse backgrounds and literacy levels, efficiently prioritizing clients’ needs.
  • Coordinate care and communicate with service providers and physicians.
  • Meet assigned productivity expectations for navigation services.
  • Submit thorough and timely documentation within 24 hours of service delivery.
  • Develop and maintain resource files and network with other service providers to assist clients.
  • Track needed data and complete benchmarking tools.

10% – Education and Outreach

  • Identify and present to community groups, places of worship, places of work and other appropriate settings about dementia education and support.
  • Build partnerships with agencies and organizations to build referral pathways.
  • Develop educational presentations for health care and senior service professionals on best practices in dementia care.
  • Be available for some evening and weekend work i.e. presentations, family consultations.
  • Support grant or funding initiatives by providing success stories and testimonials.
  • Contribute to monthly web/social media education.
  • Participate in quality improvement and staff development activities.
  • Perform other special projects and duties as assigned.
Minimum Qualifications

  • Experience working on grant/third‑party funded projects, research, or studies.
  • Experience working with people living with dementia and/or family care partners. li>
