CarePartners in Houston is seeking a full-time nurse (LVN or RN) to join our Dementia Day Center Team. The Nurse will provide direct health care, medication administration, and documentation for dementia patients, while supporting families and guiding staff in behavior interventions.

The role emphasizes compassionate, inclusive care, effective communication, and flexibility as participant needs evolve. An active Texas license is required, with LVN/RN preferred.

Qualifications

• Must hold an active LVN or RN license in Texas.
• Ability to communicate effectively one-on-one and in small groups.
• Compassion and inclusivity for individuals at all stages of dementia.

Responsibilities

• Provide daily direct health care to dementia day center participants, including medication administration.
• Complete required healthcare documentation and reporting; maintain EHR records.
• Respond to emergencies with first aid; implement dementia behavior interventions as needed.
• Educate and counsel center families; support relationships with incoming and current families.
• Guide team members in dementia care strategies and effective behavior management.
