Qualifications

Overview

We are seeking a compassionate Nurse Practitioner to join a collaborative team providing comprehensive evaluations, diagnosis, and treatment for patients living with dementia. In this fully remote role, you will work closely with an interdisciplinary team and medical leadership to develop individualized treatment plans that support patients’ needs and caregiver well-being. Position available as full-time W2 or 1099 contractor.

Locations: Florida, Texas, Washington (active NP license required)

Responsibilities
  • Deliver direct dementia care and build strong relationships with patients and families
  • Conduct cognitive, behavioral, and medication assessments
  • Participate in team meetings, case conferences, and care coordination
  • Support caregivers and assist in advanced care planning
  • Stay current with evidence-based dementia treatments
Compensation & Benefits
  • Pay range: $117,000 – $135,000
  • Choice of W2 (with benefits) or 1099 contractor

Requirements
