Job Duties – What You’ll Be Doing
• Diagnose, treat, and manage dementia and other acute episodic illnesses through comprehensive and acute care visits, promoting early detection of change, and timely treatment.
• Conduct telehealth comprehensive and acute visits with patients and caregivers.
• Perform annual visits to monitor implementation of dementia care plans.
• Facilitate appointments with consultants when treatment plans need reassessment.
• Provide dementia education and management skills to caregivers.
• Take urgent calls during daytime hours, on a rotating on-call schedule, manage or triage problems, and communicate with community primary care physicians.
• Order and interpret lab and diagnostic tests, and adjust treatment plans as needed.
• Collaborate with neurologists/dementia specialists, interdisciplinary care teams, and community PCPs to deliver quality dementia care.
• Maintain compliant documentation within our EMR.
• Collaborate with local and regional operations and clinical leadership to meet goals.
• Support strategic initiatives related to the GUIDE program.
• Provide excellent customer service to ensure high satisfaction and retention.
• Practice ethically, following the Scope and Standards of Practice, and all regulations.
• Assist in developing evidence-based training and educational materials for patients, caregivers, and public audiences.
Requirements – What We Look For In You
• Master’s of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program Graduate.
• Current certification from a recognized Nurse Practitioner credentialing body.
• Active, unrestricted APRN licensure.
• At least 2 years of experience as an APRN in primary care, internal medicine, geriatrics, neurology, or related fields.
• Knowledge of geriatrics and neurology patient care.
• Ability to take medical histories, perform examinations, assessments, and develop diagnostic and therapeutic plans.
• Timely documentation skills.
• Ability to manage a busy clinical schedule efficiently.
• Proficiency with communication and documentation platforms (EMR, Slack, email, etc.).
• Ability to assess psychosocial needs in a diverse environment.
• Knowledge of dementia assessment and treatment, including psychological, behavioral, and medical aspects.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work effectively in a fast-paced, team-based environment.
• Attend weekly clinical debriefings.
• Use care delivery and data collection technology.
• Establish cooperative relationships with patients and healthcare providers.
• Experience with electronic medical records and innovative technologies.
Preferred Qualifications
• Experience as an APRN in gerontology or dementia care.
• Clinical research experience in AD/ADRD.
• Bilingual in Spanish to support diverse populations.
