Base pay in range will be determined by applicant’s skills and experience. Role is also eligible for team-based bonus opportunities. Temporary associates are not benefits eligible but may participate in the company’s 401(k) program.

Responsibilities
Job Summary

Assists Clare Bridge Program Manager or Coordinator in leading and implementing a calendar of person-centered dementia-friendly programs that meet the specific needs of each resident within the dementia care community. Supports Brookdale’s Resident and Family Engagement philosophy by providing meaningful, person-centered programs that enhance residents’ quality of life, recognizing individual abilities and needs, and encouraging independence.

Essential Functions
  • Supports the implementation of person-centered programs tailored to individual needs and interests, engaging residents across the 7 domains of well-being.
  • Assists in reviewing resident service plans and completing program documentation as per Brookdale standards and regulations.
  • Ensures dementia care programs comply with state, federal, and other regulations, aligning with Brookdale’s mission and policies.
  • Provides guidance and resources to associates on programming and resident quality of life.
  • Coordinates and participates in community events including family socials, marketing, and open houses.
  • Communicates positively with residents, families, and community professionals about the dementia care program.
  • Supports quality improvement initiatives like the Brookdale Excellence Standards Tool (BEST).
  • Supports scheduled programming outside the community, which may include driving a community vehicle.
  • Assists with volunteer programs, including training and supervision.
  • Performs personal care, housekeeping, and dietary responsibilities as needed to maintain service quality.
Qualifications

Education and Experience: High School Diploma or equivalent required; Associate Degree preferred. Minimum one year of experience with older adults and dementia preferred.

Certifications and Licenses: Valid driver’s license and a clean 3-year driving record required.

Physical Demands and Working Conditions
  • Standing, walking, sitting, handling, reaching, climbing, stooping, talking, hearing, lifting up to 50 pounds, vision, and interaction with others.
  • Occasional weekend, evening, or night work, on-call, and exposure to communicable diseases, latex, blood-borne pathogens, chemicals, and biological hazards.
  • Travel occasionally required.

Salary Range

$12.16 - $15.20 / hour
