Goodwill of Western and Northern Connecticut in Bridgeport, CT is hiring a DDS Case Manager to coordinate and provide case management for residential program participants funded by DDS ISE. The role requires organizing service plans and coordinating benefits, with an emphasis on HIPAA compliance and CARF standards.

Ideal candidates hold a related degree or equivalent experience, possess strong communication skills, and have a valid CT driver's license.

Qualifications

• Bachelor's degree in Human Services or Psychology preferred but not required.
• Minimum 1 year of case management experience.
• Strong communication and organizational skills.

Responsibilities

• Provides case management to DDS IHS and other DDS-funded individuals.
• Assists participants in accessing community resources and benefits.
• Develops, implements, and monitors ISP service plans.
• Ensures documentation meets DDS and CARF standards.
• Coordinates medical appointments and communicates with providers.
• Maintains schedules and supports team meetings.
• Promotes community integration and HIPAA compliance.
