Job summary

Humana is hiring a Field Care Manager, Behavioral Health to assess member needs and guide families toward appropriate care resources. The role involves managing psycho-social health issues, monitoring progress, and developing care plans while following established guidelines.

Travel to Humana offices for training may be required. The position emphasizes independent judgment, patient-facing responsibilities, and collaboration with Departments of Children and Family Services where applicable.

Qualifications

• Licensed clinical social worker (LCSW), licensed professional counselor (LPC), licensed addiction counselor (LAC) or licensed marriage and family therapist (LMFT).
• Experience working with Department of Children and Family Services.
• Applicable State licensure in field of study.
• Passionate about contributing to an organization focused on improving consumer experiences.

The Field Care Manager, Behavioral Health assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Field Care Manager, Behavioral Health 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

The Field Care Manager, Behavioral Health employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensure patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. May create member care plans. Understand department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receive guidance where needed. Follows established guidelines/procedures.

Additional Information

This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$65,000 - $88,600 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
