Humana is seeking a Field Care Manager, Behavioral Health to assess member needs and guide families toward resources that support wellness. The role requires independent judgment to determine appropriate actions and may include creating member care plans.

Responsibilities include evaluating physical, environmental and psychosocial health, identifying barriers to care, and monitoring progress toward outcomes.

Qualificações

• LCSW, LPC, LAC, or LMFT licensure required or applicable in the state.
• Experience working with Department of Children and Family Services preferred.
• Managed Care experience preferred.

The Field Care Manager, Behavioral Health 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

The Field Care Manager, Behavioral Health employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensure patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. May create member care plans. LCSW, LPC, LAC, LMFTExperience working with Department of Children and Family ServicesApplicable State licensure in field of studyManaged Care ExperienceBilingual a plusWork at Home Requirements: To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: wireless, wired cable or DSL connection is suggested. Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. 65,000 - $88,600 per yearThis job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
