Job summary

Elaya Health is seeking a licensed NP or PA to join our endocrinology and diabetes care team serving skilled nursing and long-term care facilities in the Cincinnati area. You’ll provide on-site endocrine evaluations, manage diabetes and thyroid conditions, and work closely with facility nurses, primary care teams, and administration.

Qualifications

• Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Ohio
• Endocrinology experience of at least 1 year preferred
• Strong clinical judgment and communication skills
• Ability to work independently while collaborating with a larger clinical team
• Patient-centered and committed to high-quality long-term care

Responsibilities

• Perform on-site patient evaluations and follow-up visits
• Manage diabetes, thyroid disorders, and other endocrine conditions
• Review blood glucose trends, labs, and medications
• Optimize diabetes management and reduce finger sticks and burden
• Collaborate with nursing staff, DONs, facility leadership, and primary care teams
• Provide clear clinical recommendations and ongoing follow-up
• Document in facility EMRs (training provided)
• Help reduce avoidable hospitalizations through proactive specialty care

Elaya Health is a rapidly growing endocrinology practice specializing in high-quality care for patients in skilled nursing and long-term care facilities. We are seeking an experienced, compassionate Nurse Practitioner or Physician Assistant to join our expanding team.

About the Role

Our providers deliver on-site endocrine care to residents of skilled nursing and long-term care facilities throughout the Cincinnati area. You will manage patients with diabetes, thyroid disease, and other common endocrine conditions while collaborating closely with facility nursing teams, primary care providers, and administration.

What You'll Do

• Perform on-site patient evaluations and follow-up visits

Qualifications

• Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Ohio, or willingness to obtain licensure
• Experience in endocrinology (at least a year), internal medicine, geriatrics, or skilled nursing preferred
• Strong clinical judgment and communication skills
• Comfortable working independently while collaborating with a larger clinical team
