Responsibilities

• Provide incontinence care with dignity and sensitivity.
• Perform light housekeeping to maintain a safe home environment.
• Prepare meals to promote health and wellness.
• Provide medication reminders to help prevent hospital readmissions.
• Assist with safety and memory care needs as required.
• Support with physical therapy exercises when prescribed.
