Job summary

UPMC Health Plan is hiring a full-time Clinical Care Manager to support care coordination and health education for identified Health Plan members across outpatient, home health, and case management settings.

The role emphasizes face-to-face engagement with members, caregivers, and providers, developing comprehensive care plans, and coordinating with PCPs, specialists, and community resources to close gaps in preventive care and reduce hospital readmissions.

Qualifications

• Care coordination and health education for Health Plan members
• Face-to-face interactions in community, hospital, home and other settings
• Collaborates with PCPs, specialists, and care teams to coordinate care

Responsibilities

• Assist member with transition of care between health care facilities and share plan of care
• Document activities in care management tracking system
• Engage member to develop individualized plan of care with PCP and reduce ER/hospital utilization
• Coordinate and modify the care plan with member, caregivers, PCP, and specialists
• Review medication profile and address adherence issues with member and providers
