Job summary

The Home Health Aide position at ChristianaCare Health System in Newark, DE supports the clinical team under the guidance of the RN and/or occupational therapist, delivering personal care and assisting with plans of care.

Qualified applicants will have CNA or HHA training (75+ hours) and must meet ongoing competency requirements; shifts are 8:00 a.m. to 4:00 p.m. on weekdays with no weekends, and a valid driver’s license is preferred.

Qualifications

• Completion of CNA or Home Health Aide training program of 75 hours or greater.
• Applicants must have worked and received compensation as an aide for 24 consecutive months or greater since program completion.
• Ongoing competency evaluations, skills validations, and educational requirements per agency policy and regulatory requirements.

Responsibilities

• Follows Plan of Care developed by RN/Therapist.
• Adheres to infection control policies and HIPAA guidelines.
• Provides individualized personal care tasks as outlined in the aide care plan.
• Maintains a safe, clean and healthy environment for the patient.
• Encourages independence and progression toward Plan of Care goals.
• Reports observations to RN/Therapist relevant to the Plan of Care.
• Records temperature, pulse, and respiration as per plan.
