ELIZJOSEPH HEALTHCARE LLC is seeking a Home Health Aide to join our team, providing one-on-one care in patient homes with a compassionate, patient-centric approach. You will build relationships with patients, maintain detailed records, and ensure a clean, safe environment adapted to individual needs.

Ideal candidates have a Home Health Aide credential, a high school diploma or GED, and a year of related experience.

Qualifications

• Graduated from an accredited Home Health Aide program.
• High School Diploma or GED.
• One year prior professional experience.
• Driver's license required.
• CPR certification required.

Responsibilities

• Lead staff from 8:00 AM to 4:30 PM and oversee daily operations.
• Accompany clients to medical appointments and assist with insurance renewals.
• Coordinate with healthcare providers to ensure appropriate care and support.
• Monitor client progress, update records, and communicate significant changes to the team.
• Develop and implement individualized care plans.
• Facilitate communication between clients, family members, and other care providers.
