Job summary

Responsibilities

Assesses patient clinical and psychosocial needs and applies standards of practice.
Partners with patients and families to develop and evaluate a plan of care.
Facilitates patient throughput and ensures quality, ethical care.
Delivers individualized patient education and discharge instructions.
Completes all clinical and quality documentation per RHS standards and regulatory requirements.
Delegates and prioritizes tasks to support care and safety.
