Qualifications

• Applies the nursing process to plan and implement patient care.

• Collaborates with the health care team to set patient goals and monitor progress.
• Uses evidence-based practice to inform nursing decisions.

Responsibilities

• Collaborates in establishing the plan of care and coordinates delivery across the continuum.
• Analyzes information relevant to patient care to determine nursing diagnoses.
• Evaluates progress toward goals and adjusts interventions as needed.
• Promotes health and a safe environment for patients and families.
• Additional duties may be assigned as needed.
