Job summary

Claiborne Medical Center is seeking a Licensed Practical Nurse to care for subacute and long‑term residents in a nursing home setting. You will supervise CNAs, administer medications, start IVs, and perform admissions and discharge planning while maintaining safety and quality of care.

The role emphasizes teamwork, adherence to infection control, and timely communication with physicians and families. A Tennessee LPN license is required; no prior experience is necessary for this full-time day

Qualifications

• Must be licensed as an LPN in Tennessee according to the posting.
• No prior experience required for this LPN/Nursing Home role.
• Full-time position, 72 hours per pay period, day shift.

Responsibilities

• Maintains continuity of resident care during duty tours and prioritizes nursing tasks.
• Assigns residents to CNAs and supervises nursing staff; assists in planning care.
• Assists with orientation of new personnel.
• Makes rounds on residents; observes and records symptoms and changes; informs physicians and follows orders.
• Maintains good relations with visitors, families and others; relays pertinent information to responsible parties.
• Delegates authority to other licensed personnel; reports to relief staff for continuity of care.
• Starts IV; administers IV fluids and meds.
• Inserts, irrigates and feeds residents via nasogastric and gastrostomy tubes.
• Administers aerosol treatments as ordered.
• Performs admission and discharge; begins care plan within 24 hours; completes admission assessment within 8 hours.
• Administers medications (oral, IM, SubQ, topical); orders meds as needed.
• Maintains stock of drugs and supplies; ensures meds are not expired; orders stock as needed.
• Follows safety and infection control standards and policies; uses Standard Precautions.
• Supervises private sitters on units.
• Maintains dressings and wound care; monitors wounds and notes changes.
• Compares current care with plan of care and completes monthly summaries.
• Orders treatments online (labs, x-rays, diets, etc.).
• Ensures staffing coverage in absence of Nursing Supervisor/DON.
• Ensures two licensed persons sign physician orders and documents verbal orders.
• Maintains a safe environment; reports abuse or neglect as required.
• Demonstrates age-specific competencies; participates in quality improvement.
• Performs other duties as assigned.
