Job summary

Farmington Wellness & Rehabilitation is seeking a dedicated LVN to lead nursing care on day shifts in a 24/7 skilled nursing environment. You will oversee staff, administer medications, and coordinate with physicians to ensure high-quality resident outcomes.

The role requires an LVN license, 2+ years in SNF, and current BLS/CPR. You will work under the DON and collaborate with a multidisciplinary team to maintain regulatory compliance and resident satisfaction.

Qualifications

• Graduate of an approved LVN program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
• Current BLS/CPR certification.

Responsibilities

• Conduct daily nursing functions in accordance with company, state, federal and local rules and guidelines.
• Manage shift staffing, assignments, mentoring and education of nursing personnel.
• Administer medications and transcribe/execute physician orders as allowed by the Nurse Practice Act.
• Collaborate with physicians and other medical professionals to provide resident care.
• Ensure compliance with the MDS process and PCC system as part of care planning.
