Qualifications

• Must be licensed as LPN in Tennessee.

Responsibilities

• Provide direct patient care to residents, including administering medications, wound care, and other treatments as prescribed by physicians.
• Develop and implement individualized care plans in collaboration with the multidisciplinary team.
• Conduct routine assessments and monitor patient progress to ensure optimal outcomes.
• Collaborate with dietary and activities staff to develop and implement patient-centered care plans.
• Maintain accurate and up-to-date patient records and reports.
• Participate in quality improvement initiatives to optimize patient care and outcomes.
