Job summary

Maple Health and Rehab, Muhlenberg County, Kentucky, is seeking LPNs and RNs for a full-time day-shift role to deliver personalized resident care in a facility with a dedicated memory care unit and strong staff ratios.

You will coordinate with the healthcare team to plan and evaluate care, administer medications, monitor vital signs, and participate in discharge planning. We offer mentorship, growth opportunities, and a comprehensive benefits package.

Qualifications

• Current RN/LPN license in the state.
• Up-to-date first aid and CPR certification.
• At least one year of LPN or RN experience in a long-term care setting is preferred.
• Knowledge of MatrixCare preferred.

Responsibilities

• Assess, plan, implement, and evaluate patient care plans with the healthcare team.
• Administer medications and record prescribed treatments; report adverse reactions.
• Record patients' medical information and vital signs.
• Monitor equipment and interpret electronic displays.
• Participate in discharge planning.
• Provide bedside care and assist physicians during examinations.
• Initiate corrective action for adverse symptomatology.
• Manage time and document care accurately.

LPN/RN Core Responsibilities:

• Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans
• Prepares and administers (orally, subcutaneously, through an IV) and records prescribed medications. Report adverse reactions to medications or treatments in accordance with the policy regarding the administration of medications by a licensed registered nurse.
• Records patients' medical information and vital signs.
• Monitors and adjusts specialized equipment used on patients, and interprets and records electronic displays.
• Initiates corrective action whenever the patient displays adverse symptomatology
• Provides bedside care for a wide variety of medical patients.
• Changes dressings, inserts catheters, and starts IVs
• Prepares equipment and aids the physician during the examination and treatment of a patient
• Responds to life‑saving situations based on nursing standards and protocol
• Participates in discharge planning.
• Utilizes time management skills.
• Records all care information concisely, accurately, and completely, in a timely manner, in the appropriate format, and on the appropriate forms.
• Performs other position‑related duties as assigned, depending on the assignment setting.

LPN/RN Requirements:

• Current RN/LPN license in the state
• Up-to-date first aid and CPR certification
• At least one year of LPN or RN experience in a long‑term care setting is preferred.
• Knowledge of MatrixCare preferred
