** COMPANY DESCRIPTION**

Compassionate Services and Consulting (CSC) is a social services agency based out of Omaha, providing residential and day program services to participants in Nebraska with intellectual and developmental disabilities. Compassionate Services and Consulting strives for the highest degree of ethical person-centered practices to best serve one of our community’s most vulnerable population! ** SUMMARY OF MAIN RESPONSIBILITIES**

Day Coordinator position manages the day-to-day supervision and communication with Direct Support Professional(s) (DSP). Manages the individual in services progress and ensures that supervision is appropriate. Program writing, monthly goal reviews, and meeting attendance for individual's ISPs are included duties for the position.

  • Supervise Lead DSPs and DSPs
  • Manage staff schedule
  • Write and review Individual's BSP/Hab plans
  • Attend and lead ISP meetings

** Specific Responsibilities**

Compassionate Services and Consulting, through quality, integrity, innovation, and with a growth mindset, builds communities where people with disabilities achieve their goals.

  • Ensure compliance with agency policies and procedures, contractual agreements established with Nebraska’s Department of Health and Human Service System, as well as ensuring compliance with state, federal and local regulations and laws.
  • Ensures client rights are maintained and respected via Day Services floor supervision, GER audits, and attendance at quarterly Human Right Committee meetings.
  • Collaborates with Compassionate Services staff, individuals, and individuals' team to ensure client specific protocols, medications, or medical needs are in place and are adhered to by Day Staff.
  • Collaborates with CEO to ensure client safety plans, behavioral support plans, behavior trackers are person-centered, implemented, and required documentation completed.
  • Quality Assurance of data collection and client records.
  • Monthly audits and updates client specific habilitation & behavioral programming.
  • Completing monitoring activities and providing technical assistance to verify the accurate and timely completion of required documentation.
  • Attending Individual Support Planning meetings
  • Write quality programs on or before the ISP meetings/intakes. Development of programming by conducting behavioral and skill assessment of clients though behavioral assessments to identify and access individual strengths and needs.
  • Review GERs within 24 hours of submission and communicate with Services Director when GER is ready to approve.
  • Creating habilitative programs and activities for the individuals assigned to the coordinator's caseload.
  • Function as a member of a treatment team for adults in services. This includes working with a Therapists, guardians, physicians, and DHHS workers. Work in collaboration with schools, providers, mental health professionals, and other community resources.
  • Ensure day services schedule is maintained and followed, encouraging meaningful activities and community centered activities.
  • Conduct training sessions based on staff and organizational learning needs. Includes assistance in development, implementation, and continuous improvement of onboarding training program and continued training for current employees.
  • Maintain open communication with staff through staff meetings.
  • Collaborate with DHHS Service Coordination for visits with individuals in services. Includes communication of program changes, attending ISP meetings, notification of incidents, and contact for referrals.
  • Function as a member of a treatment team for both adults and children in the system. This includes working with a Therapists, guardians, physicians, DHHS workers, and child and family services professionals. Work in collaboration with schools, providers, mental health professionals, and other community resources.
