Job summary

CHOICE, Incorporated seeks Direct Care staff in Maple Grove, MN to support individuals with intellectual disabilities. The role focuses on education, enrichment, and community integration, with coaching and individualized training for participants in community and center settings.

You will provide health-related care per procedures, transport participants when needed, and help them achieve daily goals. A valid driver’s license and 1+ year experience required.

Qualifications

• Bachelor’s degree (BA/BS) strongly preferred, Human Services-related field recommended.
• Associate’s degree plus 2 years of related experience may substitute for a 4-year degree.
• At least 1 year of direct care experience is required.
• Valid driver’s license with clean record.
• Ability to bend, stoop, reach and lift at least 40 pounds.
• Professional demeanor and strong communication skills.

Responsibilities

• Plan and implement educational and recreational activities and programs in CHOICE centers.
• Provide job coaching and individualized training for participants in community settings.
• Support participants to access and participate in community-based activities, including transport.
• Follow plans to achieve daily or weekly goals and collect data for evaluation.
• Perform basic health, medical and personal care procedures as required.
