You will follow individual service plans, monitor health and safety, assist with daily living tasks, transport when needed, and document progress in logs and reports while collaborating with a caring, dedicated team.

Qualifications

• Must be at least 18 years old.
• GED or High School Diploma required.
• Compassionate, dependable, and team-oriented.
• Strong communication skills and willingness to learn.
• Valid driver’s license.

Responsibilities

• Lead structured activities including social, recreational, and community-based programming.
• Support individuals in building independence, social skills, and daily living skills.
• Implement individual service plans and track progress toward goals.
• Ensure health, safety, and dignity during program hours.
• Provide personal care and administer medications as trained.

• Complete required documentation, logs, and incident reports.

What You’ll Do as a Day Program Professional

As a Day Program Professional, you will support individuals in a structured daytime setting, leading engaging activities, building skills, and promoting independence and community inclusion.

Daily Responsibilities

• Lead structured activities including social, recreational, and community-based programming
• Support individuals in building independence, social skills, and daily living skills
• Implement individual service plans and support progress toward personal goals
• Ensure health, safety, and dignity of individuals during program hours, transportation, and community outings
• Provide personal care and administer medications as needed (training provided)
• Promote inclusion and positive community engagement
• Utilize crisis prevention and intervention strategies to support individuals safely
• Complete required documentation including daily logs and incident reports
• Transport individuals and supervise activities in community settings
• Prepare meals and maintain a clean, organized home environment
