Job summary

American Training Inc. is hiring a Case Manager for the Day Program in Lowell, MA.

The role focuses on maintaining confidential guest files, developing ISP/DHSP plans, and coordinating with families, providers and clinicians to support guest goals and safety. The ideal candidate has at least one year in the developmental disabilities field, with a willingness to learn and strong written and interpersonal skills.

Qualifications

• At least one year of experience in the Developmental Disabilities field.
• If no Degree, at least 2 years of experience in the field.

Responsibilities

• Maintain Guest Confidential Files in compliance with funding requirements.
• Write ISP and DHSP goals, objectives and implementation strategies.
• Attend ISP, IDT and ABI meetings and provide leadership.
• Serve as liaison to families and residential providers for scheduling and other needs.
• Assist with guest referral process and collection of medical records.
• Maintain a clean Case Management area and support guest safety during drills and emergencies.
• Record quality data and monthly reporting; participate in staff trainings.
