• Facilitate an individual's independence, inclusion and productivity (the 3 IP's).
• Complete daily checklists and monthly summaries based on Day Hab goals.
• Support and advocate for individuals within the community.
• Create meaningful activities aligned with personal goals.
• Performs all other duties as assigned.
