Job summary

RISE LTD in Elkader, IA is seeking a Day Habilitation Direct Support Professional for our Day Habilitation Elkader program. You will directly support individuals to increase life skills and reach personal goals in a community-based setting.

Role involves planning activities, assisting with daily living skills, coordinating transportation for community resources, and documenting progress, with some medication passes and flexible hours. Training and supportive supervision provided.

Responsibilities

• Day Hab activities are scheduled 4 times per day.
• Researching and planning activities to promote life skills (coping, community navigation, healthy lifestyles, cooking, cleaning, personal cares).
• Passing medications. Typically 3-5 medication passes per day.
• Transportation of clients to community resources, activities, and volunteer opportunities at least 4 times per month.
• Documenting time spent with clients, this typically takes half an hour per day.

Various locations in the community while providing community based services.

Schedule

Monday-Friday 7:00am-2:00pm (Possibility of evening and weekend hours due to group events)

Some flexibility - Documentation may be completed at home after 1:30pm

Accept changes in work schedule/flexible with hours

Requirements

The Day Habilitation Direct Support Professional is responsible for providing support directly to the individuals in RISE Ltd.’s Day Habilitation program. This person is responsible for assisting to increase the individual's life skills and meeting their personal goals.

• Day Hab activities are scheduled 4 times per day.
• Researching and planning creative, fun, and new activities to promote life skills. (coping skills, navigate in the community, promoting healthy lifestyles, cooking, cleaning, personal cares, problem solving, money management, rights and responsibilities, recognizing social cues, technology skills, volunteering opportunities)
• Passing medications. Typically 3-5 medication passes per day.
• Transportation of clients to community resources, activities, and volunteer opportunities at least 4 times per month.
• Documenting time spent with clients, this typically takes half an hour per day.
