Job summary

Community Options, Inc. in Queens, NY is seeking a Day Habilitation Coordinator to oversee billing documentation, medication administration, and document consumer progress toward established ISP goals.

• Provide ongoing supervision and support for community integration.
• Monitor and report health and medical needs of individuals.
• Document daily progress toward ISP goals.

We are now hiring a dedicated Day Habilitation Coordinator in Queens, NY. The Day Habilitation Coordinator is responsible for the oversight of billing documentation, medicationadministration, and documenting consumer daily progress towards established goals

Starting pay is $70,000/ per year

Responsibilities

• Responsible for the oversight of billing documentation in a timely manner and verifying for compliance
• Complete staff schedules and approve payroll in UKG
• Review Therap daily to ensure all T-Logs and ISP data are entered daily and follow up with staff to get documentation completed and filed monthly
• Input/update/discontinue all staff action plans and IPOP’s
• Attend all IDT meetings
• Provide on‑going supervision and support for community integration
• Monitor and report health and medical needs of individuals
• Document individual daily progress toward completing established goals, based on ISP
• Assist with filling open direct support professional shifts and cover them, as necessary
• Employee must cooperate with the license and department staff in any inspection, inquiry, or Investigation

Minimum Requirements

• Valid Bachelor's degree
• 2+ years of related work experience
• Ability to present facts and recommendations effectively in oral and written form
• Valid driver’s license
• Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing
