The position requires a Bachelor’s degree in Education or a related field and at least two years’ experience with developmental disabilities, plus a valid NY driver’s license.

Qualifications

• Bachelor’s degree in Education, Social Work, Human Services or related field required.
• Two years’ experience in working with people with developmental disabilities preferred.
• Valid NYS driver’s license and clean driving record required.
• Demonstrated organizational and interpersonal skills.
• Proficient in computer software and programs; strong written and oral communication.

Responsibilities

• Develops meaningful partnerships with individuals, families, and the community.
• Provides support and encouragement to individuals to determine their pathway in life.
• Advocates for people’s right to make meaningful choices.
• Attends Life Plan reviews and updates plans to meet regulatory guidelines.
• Trains staff on daily documentation and individualized action plans.
• Reviews monthly reports and conducts functional assessments.
• Presents a professional image with individuals, families, staff, and community members.
• Maintains confidentiality of protected health information and participates in audits.
