Job summary

Immanuel Pathways PACE® seeks a Participant Care Aide to serve at Pathways Southwest Iowa in Council Bluffs, IA. The role includes personal care, ADLs, and supportive care in center or participant homes with a Monday–Friday 8:00 am–4:30 pm schedule and an 8th weekend rotation.

Salary is up to $22.06 per hour, based on experience, certifications, and education. Immanuel offers comprehensive benefits including health, retirement, PTO, and growth opportunities.

Qualifications

• High school diploma or equivalent is required.
• Active Certified Nursing Assistant Certificate required in the respective state.
• Be at least 18 years of age.
• One year of experience working with the frail or elderly population required, or completion of job specific training related to working with the elderly population must be completed within the first six months of hire.
• Must have a valid driver’s license, proof of insurance and have your own reliable vehicle.

Responsibilities

• Provide personal cares and activities of daily living to participants in center or home settings.
• Monitor participants' health status and maintain a safe environment.
• Assist in coordinating in-home care as needed and participate in restorative/recreational activities.
• Document care activities and report changes to the care team.
• Follow program policies and safety guidelines to ensure quality care.
