Responsibilities

• Perform plan of care interventions including meds and clinical treatments, documenting care.
• Monitor and communicate patient status with care team, physicians, patients and families.
• Coordinate care across disciplines; assist with transfers, discharges, referrals, and psychosocial support.
• Educate patient and family appropriate to age, culture, condition, and circumstances.
• Advocate for the patient’s physical and emotional well-being.
