Qualifications

  • Bachelor's or Master's degree as applicable with clinical experience in mental health.
  • Experience in crisis intervention, case management or in home therapy preferred.
  • Phone triage experience and ability to perform psychiatric assessments are desirable.

Responsibilities

  • Coordinate care for a caseload of 10–15 individuals/families.
  • Support goal setting, crisis planning, and WRAP development.
  • Provide counseling, advocacy, and follow-up services in community and office settings.
  • Assist with access to food, healthcare, education, employment, and housing.
  • Facilitate small groups to build coping and daily living skills.
  • Conduct crisis assessments and manage emergencies.
  • Collaborate with ER/DEC staff to divert from inpatient care.
  • Maintain accurate records and meet documentation deadlines.

Job description

UPMC’s Western Behavioral Health’s Community Treatment Team (CTT) provides intense outpatient mental health treatment services to consumers with serious and persistent mental illnesses who have not previously found success in traditional outpatient mental health services. Our clinicians provide community-based, evidence-driven care for individuals with severe mental illness, offering support through assessments, crisis response, and outpatient coordination. This 24/7 role is dynamic and rewarding, with opportunities for growth, teamwork, and professional development. As the first licensed ACT team in PA, our program is recognized as “Exemplary” in annual reviews.

Responsibilities
Client Support & Case Management
  • Coordinate care for a caseload of 10–15 individuals/families.
  • Support goal setting, crisis planning, and WRAP development.
  • Provide counseling, advocacy, and follow-up services in community and office settings.
Daily Living & Community Integration
  • Assist with access to food, healthcare, education, employment, and housing.
  • Facilitate small groups to build coping and daily living skills.
Crisis & Collaboration
  • Conduct crisis assessments and manage emergencies.
  • Collaborate with ER/DEC staff to divert from inpatient care.
  • Work with team and community partners to coordinate services.
Documentation & Compliance
  • Maintain accurate records and meet documentation deadlines.
  • Ensure compliance with institutional and ACT model standards.
  • Meet a 55% productivity rate.
Professional Development & Availability
  • Participate in on-call and weekend rotations.
  • Apply motivational interviewing and evidence-based practices.
  • Engage in ongoing ACT training and implementation.
  • Bachelors Degree in Psychology, Counseling or Social Work with three years clinical experience
OR
  • Masters Degree in Psychology, Counseling, or Social Work required.
  • Other fields of study may be considered with coursework of at least 12 credits in human services and/or relevant mental health experience.
  • Work history that includes phone triage, as well as an in depth knowledge and proven skill at doing psychiatric assessments.
  • Experience in mental health crisis intervention, case management, or in home therapy preferred.
  • Excellent, recovery-oriented interpersonal relations.
  • Ability to work shifts, weekends, holidays, and to assume on call responsibilities.
  • May in the course of their duties be in rooms where medications are stored or may transport medications when they transport patients.
  • Transportation of medication is limited from one department to another
