Responsibilities

• Performs procedures, monitoring, or other functions as ordered by the medical provider(s) and documents care in the patient record.
• Educates patients, families, and caregivers about condition, status, treatment, medications, side effects, and follow-up measures; translates complex terms.
• Educates patients on non-medicinal follow-up measures, such as diet, exercise, disease prevention, and self-management.
• Implements evidence-based practices and process improvements to meet quality measures and enhance patient care outcomes.
• Collaborates with a multidisciplinary team to coordinate and deliver a holistic patient treatment plan.
