Responsibilities

• Coordinate care for patient/family needs across the care continuum.
• Discharge planning, scheduling services, and monitoring care delivery.
• Advocate for patients and evaluate outcomes with the care team.
• Ensure unit safety, equipment operation, and accurate charting.
• Participate in mandatory training and unit meetings.
