Job summary

Concord Hospital Health System seeks an Advanced Practice Nurse Practitioner to provide inpatient pulmonary and critical care services for adults with acute and complex illnesses. You will work with Pulmonary/Critical Care physicians, perform comprehensive assessments, participate in ICU rounds, and coordinate care across disciplines.

The role requires ACNP certification and current New Hampshire licensure or eligibility, ACLS, and prescriptive authority.

Qualifications

• ACNP certification and NH licensure (or eligibility)
• Master’s degree or higher in Nursing from an accredited program
• ACLS certification and DEA registration with prescriptive authority
• Critical Care/ICU RN experience required
• APRN with CCU/ICU specialization preferred

Responsibilities

• Perform patient assessments, histories, and physical exams for adults with acute and complex conditions
• Lead daily ICU rounds and inpatient pulmonary consultations
• Develop and manage treatment plans for pulmonary and critical care conditions
• Order and interpret diagnostics and coordinate multidisciplinary care
• Prescribe and manage medications within scope of practice
• Collaborate with physicians, nursing, respiratory therapy, and consultants
• Provide patient and family education and discharge planning
• Ensure timely EMR documentation and participate in quality improvement initiatives
• Coordinate admissions, transfers, and follow-up care
