UnityPoint Health is seeking a Crisis Services Case Manager for a community-based role in the Des Moines area. The role focuses on providing case management for Mobile Crisis Response Team (MCRT) and Crisis Stabilization Community-Based Services (CSCBS).

Expect outreach, client file maintenance, on-call coverage, and collaboration with community partners. The position is full-time, Monday–Friday, 8:00 a.m.–4:00 p.m., with on-site work in Des Moines, IA.

Qualifications

• Must meet one of the listed education levels.
• Experience varies by education level: Mental health professional 1 year; Bachelor’s 1 year; Associate 2 years; High school diploma 2 years.

Responsibilities

• Provide case management services for clients per program policies.
• Complete required paperwork and maintain client files.
• Develop and maintain relationships with mental health professionals, law enforcement, schools, hospitals and community partners.

Overview

Meet virtually with our team and learn how you can support individuals experiencing mental health crises in their communities.

This position provides case management for the Mobile Crisis Response Team (MCRT) and Crisis Stabilization Community-Based Services (CSCBS) program. Responsibilities include conducting outreach and presentations on available services, maintaining client files, covering assigned on-call MCRT day shifts, and providing support and consultation that encourages recovery-focused behaviors for individuals participating in a mental health program.

Employment option: Full-time, day shift

Current openings: Des Moines, Iowa. This is a community-based role that includes client outreach and scheduled MCRT daytime on-call coverage.

Shift: Monday–Friday, 8:00 a.m.–4:00 p.m.

What You Will Do

Provide case management services to clients, maintain accurate client files and documentation, build working relationships with mental health practitioners, law enforcement, schools, hospitals, and community partners, share information about available crisis services through outreach and presentations, and cover scheduled MCRT day shifts.

Qualifications

Education

Must meet one of the following:

• Mental health professional as defined in Iowa Code section 228.1
• Bachelor’s degree; or
• Associate’s degree; or
• High school diploma

Experience (based on education level noted above)

• Mental health professional: 1 year of mental health experience
• Bachelor’s in human services: 1 year
• Bachelor’s in non-human services: 2 years
• Associate’s degree: 2 years
• High school diploma: 2 years

Licenses/Certifications

• Valid driver’s license and compliance with Eyerly Ball’s auto insurance policy
• Must pass criminal records and dependent adult/child abuse checks
