Qualifications

• High school diploma or equivalent.
• Bachelor’s degree in social work, human services, or related field.
• Case management experience.
• Bilingual in Spanish / English.

Responsibilities

• Assessment activities: conduct assessments and develop an individualized family service plan.
• Case management activities: support families to maintain children in their homes and manage crisis situations.
• Community liaison: coordinate with referral sources and ensure distribution dates are staffed.
