Job summary

Bay Cove Human Services in Massachusetts seeks a CCS Mental Health Worker to provide direct crisis services for adults in the CCS. You will manage admissions, treatment planning, daily documentation, and discharge with 1:1 supervision as needed.

Responsibilities

• Completes and documents admissions, treatment plans, client daily documentation, and discharge summaries in accordance with BCHS, DMH, and regulatory standards.
• Ensure unit follows daily schedule, supervise therapeutic groups as leader or observer.
• Maintain unit cleanliness, assist in meal preparation and laundry, arrange bed setup and linens.
• Maintain relationships with insurers, DMH, and community resources for care coordination.
• Participate in daily rounds and follow up on issues for clients.
• Adhere to safety protocols and facility safety measures.
• Support referrals and warm handoffs to CBHC services as directed by clinical team.
• Document in eHana and communicate information for admissions, discharge, and follow-up workflows.
• Participate in training, team consultation, and quality improvement activities.
• Perform other duties as assigned.
