Qualifications

Responsibilities

• Provide short-term stabilization plans and crisis intervention.
• Coordinate services with multiple providers and referral sources.
• Maintain accurate records and documentation in electronic health record system.
• Provide client education and advocacy to connect with community resources.
• Assist with mobile outreach and services in homes and community settings.
