Responsibilities

• Conduct assessments, diagnose, formulate treatment plans and monitor interventions.
• Perform case management and utilization review and patient education.
• Coordinate referrals within hospital and Aurora Behavioral Health Services for continuity of care.
• Engage in supervision, peer review and professional consultations.
